Developed by Stanford University's Patient Education Research Center, the "Living a Healthy Life with Chronic Conditions" program is a discussion-based, self-management workshop designed to empower adults living with chronic conditions.This workshop is designed to help participants discover strategies to improve their quality of life while managing their symptoms. Learn how to set healthy goals, make action plans, manage pain, embrace nutrition and exercise, understand medication, increase your confidence in managing your health, plus more!This program consists of six (6) weekly, 2.5 hour sessionsIt is presented in partnership with Oasis and Aging Ahead.Through EDUCATION, ADVOCACY, and relevant PROGRAMS, the Oasis impact reaches communities in St. Louis and beyond.Aging Ahead provides PROGRAMS, SERVICES, and RESOURCES to support individuals and their caregivers on the journey of aging.
